Beyond Cortisone: Smarter, Longer-Lasting Solutions for Shoulder Pain

If you’ve been dealing with shoulder pain for a while, there’s a good chance cortisone injections have come up as an option. And it makes sense. When pain is limiting your sleep, workouts, or even simple daily tasks, fast relief sounds appealing. 

But one of the most common things we hear from patients at Penrose & Associates Physical Therapy is this: “The shot helped… but only for a little while.” We’ve worked with many individuals who found meaningful, lasting relief without ever needing a cortisone injection. And for those who have tried it, the relief is often temporary because the root cause of the problem hasn’t been fully addressed. 

What Cortisone Does (and Doesn’t Do) 

Cortisone is designed to reduce inflammation, which can ease pain fairly quickly. That can be helpful in certain situations, especially when symptoms are more severe. But cortisone doesn’t fix how your shoulder is moving, stabilizing, or handling stress. While it may calm things down temporarily, the same issue often builds back up over time. 

In some cases, repeated use may also slow the body’s natural healing response in tendon tissue. The key takeaway is that it can reduce pain, but it doesn’t rebuild a stronger, more resilient shoulder. 

Why Pain Keeps Coming Back 

Shoulder pain is rarely just about inflammation. It is usually about how the shoulder is functioning as a system. When the rotator cuff, shoulder blade, and surrounding muscles are not working together well, certain tissues take on more stress than they should. 

Over time, that is what leads to irritation, pain, and recurring symptoms. If that underlying pattern does not change, the pain often returns, regardless of short term relief. 

A More Effective Path to Recovery 

The encouraging news is that shoulders respond very well to the right kind of treatment. Instead of only focusing on pain relief, we focus on restoring how your shoulder actually works. 

Treatment at our clinic typically includes hands on care to improve mobility and reduce irritation, targeted strengthening to build support and stability, and movement retraining to reduce strain and improve coordination. As your shoulder starts moving and functioning better, pressure on irritated tissues decreases, and that is when more consistent, lasting relief begins to happen. 

Supporting Healing from the Inside Out 

In most cases, we also use regenerative therapies to help accelerate recovery. Shockwave therapy helps stimulate blood flow, tissue repair, and tendon healing. EMTT works at a deeper level to reduce inflammation and support cellular recovery. Dry needling can relieve tension and improve muscle activation.
