Dizziness After 50: Common, But Not “Just Aging”

If you’ve ever rolled over in bed and felt the room spin, or looked up to reach a cabinet and suddenly had to grab the counter, you’re not alone.

One of the most common causes of vertigo in older adults is Benign Paroxysmal Positional Vertigo (BPPV).

Inside your inner ear are tiny calcium crystals that help your brain sense gravity and movement. With BPPV, some of these crystals become displaced and move into areas where they don’t belong. When you change head position, the loose crystals shift and send incorrect signals to your brain, creating a spinning sensation even though you are still.

Common BPPV symptoms include:

  • Spinning with position changes (rolling in bed, looking up, bending forward)
  • Brief but intense episodes lasting seconds to minutes
  • Nausea or a “washed out” feeling afterward
  • Imbalance between episodes

BPPV is mechanical, not psychological. That means it often responds very well to specific mechanical treatment.

BPPV Is Highly Treatable

A key point many people never hear: BPPV is often highly treatable.

Trained vestibular physical therapists use precise head and body positioning techniques to guide the crystals back where they belong. These maneuvers are selected based on which ear and canal are involved.

During treatment, your therapist will:

  • Position you in a specific sequence
  • Monitor your eye movements to confirm the diagnosis
  • Move carefully to keep you safe and supported

Many people improve in just a few focused sessions. However, not all dizziness is BPPV, which is why a thorough evaluation is essential.

Not All Dizziness Is BPPV

Other contributors to dizziness and imbalance may include:

  • Vestibular hypofunction (reduced inner ear function)

  • Visual dependency (feeling off-balance in busy environments)
  • Neck dysfunction
  • Sensory changes such as neuropathy

Medication may temporarily reduce symptoms, but it does not correct mechanical or sensory issues. Identifying the true cause is the first step toward lasting improvement.
